Upload Immunization Records

All students attending Coastal Carolina University are required to meet the University's immunization requirements prior to registration; a copy of your immunization record must be provided to Student Health Services prior to your enrollment. 

Immunization records must be signed and verified by a physician, physician assistant, nurse practitioner, or stamped by the health care facility. Your immunization record must include the month, day and year vaccine was given. All immunization records must be submitted in English. (These are typically the same records that students would need to submit to their high school for immunization compliance. Most students should already have possession of these records.)

Please allow 24-72 hours for your immunization records to be verified after submission.

How to Submit Immunization Documentation:

  1. Go to https://coastal.medicatconnect.com and log in with your CCU username and password.
  2. Click on the “Upload” tab on the left side of the screen.
    • Click "Select File" for the document type that you are wanting to upload.
    • Select your file and click the "Upload" button.
  3. Click on the “Immunizations” tab on the left side of the screen
    • Click on "All Required"
    • Select the Immunization type from the drop down list.
    • Enter the date or dates that the immunization was administered and click "Submit"
    • Repeat this process for each required immunization.
  4. Click on the “Checklists and Forms” tab on the left side of the screen.
    • Click on the "TB Risk Assessment" link in the *Required Immunization Documentation section.
    • Complete the TB Risk Assessment form and click "Submit"
    • Only complete the Meningitis Waiver Form if you have not received the Meningitis vaccine and do not intend on receiving the vaccine before your immunization information is due.
